Who can use this form
Use this form if you are the employer that made the variation of the agreement.
Before you start
Have this information ready before you begin. You will need to tell us:
- your details as the person making the declaration
- information about the affected employees
- any material you gave the affected employees about the proposed variation of the agreement
- information about the steps you took to inform the affected employees about how and when to vote on the variation
- the outcome of the vote.
You must send (serve) a copy of your declaration to each employee organisation that is covered by the agreement.
