Employer’s declaration in relation to a variation of a single interest employer agreement to add an employer and employee (Form F23FA)

Fill in parts 1, 2 and 4 if you are an employer (or are completing this form for an employer) that has lodged a Form F23F and you need to lodge a declaration in support of that application.

OR

Fill in parts 1, 3 and 4 if a union that has lodged a Form F23G and you need to lodge a declaration about that application on behalf of an employer.

Have this information ready before you begin. You will need to tell us:

• any material you gave employees about the variation of the agreement
• details of the voting process including dates and steps in the process
• the varied agreement and how you explained the terms of the agreement to employees
• information you gave employees about when and how to vote
• whether the varied agreement will cover employees in general building and construction work

You must send (serve) a copy of your declaration to each employer and employee organisation that is covered by the agreement.
