Use the Form F23EA declaration in relation to a variation of a supported bargaining agreement to add an employer and employees.

Who can use this form
Fill in parts 1, 2 and 4 if you are an officer or authorised employee of a union that has lodged a Form F23E and you need to lodge a declaration in support of that application.
OR
Fill in parts 1, 3 and 4 if an employer has lodged a Form F23D and you need to lodge a declaration in relation to that application on behalf of a union.
Before you start
Have this information ready before you begin. You will need to tell us:
- the number of employees employed by the employer
- the industry of the employer
You must send (serve) a copy of your declaration to each employer and employee organisation that is covered by the agreement.
