Declaration of employee organisation in relation to an employer’s application for approval of a variation of a cooperative workplace agreement to add an employer and employees (Form F23IB)

Use this form if you are an employee organisation covered by the agreement and you have received the Form F23IA declaration in relation to the varied agreement.

Have this information ready before you begin. You will need to tell us:

• your details
• the name of the agreement that will be varied if approved
• the name of the employer that will be covered by the agreement
• details of any statements in the Form F23IA that you disagree with, and why.

You must send (serve) a copy of your declaration on the employer that will be covered by the agreement and each employee organisation that is covered by the agreement.
